Too often, companies set internal goals like “no nonconformances from external audits.” On paper, this sounds great — but in practice, it can encourage a culture of concealment, where issues get buried rather than tackled head-on.

There were multiple times I’d arrive to audit a site and ask to review any incident data — whether for quality, health & safety, or the environment — only to be told "we’ve had no incidents in the past 12 months."

If true, that’s a fantastic achievement. But most of the time, it didn’t quite stack up.

Evidence would surface:

  • Quarantine areas full of “on hold” stock

  • Ransacked first aid boxes

  • Damaged racking and unsafe equipment

  • Conversations with employees telling a different story

These aren’t signs of failure — they’re signs of opportunities being missed because of a reluctance to acknowledge what isn’t perfect.
