Most ISO 9001 nonconformities are not caused by poor intentions. They usually occur because systems are not reviewed regularly, responsibilities are unclear or processes have drifted over time.

The purpose of an audit is not to catch people out. A good audit should provide confidence that processes are working effectively whilst identifying opportunities for improvement.
